1. Purpose
The Provider agrees to offer complimentary laser skin treatments to the Model in exchange for the Model’s participation in photo/video documentation and testimonial sharing for marketing purposes.
2. Model Commitments
- The Model agrees to:
- Attend all scheduled treatment sessions on time.
- Inform the practitioner of any changes in health, medications, or lifestyle that may affect suitability.
- Provide a full written testimonial within 14 days after the final session.
- Allow photographs and/or videos to be taken before, during, and after treatments.
- Grant the Provider unlimited rights to use photos, videos, and testimonials for marketing purposes (social media, website, print materials, online/print marketing, educational content, and training materials).
- Content may be edited, adapted, or combined with other media, without further approval from the participant.
3. Provider Commitments
The Provider agrees to:
- Deliver the agreed laser treatments free of charge.
- Provide safe, professional treatments according to industry standards.
4. GDPR Consent & Data Handling
- The Model consents to the Provider collecting and storing their personal data (e.g., name, contact details, treatment notes, images, and videos) for the purposes of providing treatments, and using media for marketing and promotional activities.
- Data will be stored securely in compliance with UK GDPR and will not be shared with third parties other than for agreed marketing purposes.
- The Model understands they can withdraw consent for marketing use of images/videos at any time by contacting the Provider in writing, though this will not affect media already published before withdrawal.
- Data will be retained for as long as necessary for marketing purposes or until consent is withdrawn.
5. Image & Media Release
The Model grants the Provider unlimited, royalty-free, worldwide, irrevocable rights and license to use and publish images, videos, and testimonials for promotional purposes and in marketing materials without further compensation.
5.1 Third-Party Content Use
From time to time, the Provider may collaborate with approved third-party professionals (such as photographers, videographers, or marketing partners) to capture and produce content during treatment sessions.
By signing this agreement, the Model consents to these collaborators capturing, editing, and using agreed photos or videos for the purpose of promoting their own professional services, in addition to the Provider’s marketing use. All collaborators will be required to use the content responsibly and in a manner consistent with the Provider’s professional standards. No additional payment will be due to the Model for this use.
6. Cancellation & Penalty Clause
- Appointments must be cancelled or rescheduled with 48 hours’ notice.
- Failure to attend a session or cancel a session with less than 48 hours’ notice may result in termination of this agreement and cancellation of future treatments.
- If the Model fails to attend a session or does not supply the testimonial within 14 days of their final treatment, the Provider reserves the right to invoice the Model for the full and total cost of the entire treatment package and all sessions included, even if all sessions weren’t attended.
7. Liability Waiver
- The Model acknowledges that laser treatments carry risks (such as redness, irritation, pigmentation changes) as laid out in the pre-treatment consultation forms and agrees to follow all pre- and post-care instructions provided.
- The Provider is not liable for any adverse reactions that occur from failure to follow these instructions.
- Results vary per individual; no guarantees are made regarding outcomes
- Participation in the Programme is voluntary, and the clinic/practitioner shall not be liable for any dissatisfaction with cosmetic results.
8. No Monetary Compensation
The Model understands that the treatments provided are the sole compensation and that no additional payment will be made.
9. Agreement Duration
This agreement remains in effect until treatment plan completion or early termination for non-compliance.
10. Right to Withdraw
10.1 Provider’s Right to Withdraw
The Provider reserves the right to withdraw a participant from the Programme at any time if they:
- Fail to attend scheduled appointments.
- Withdraw or refuse consent for photo/video/review use after treatment has begun.
- Act in a manner that disrupts the treatment or filming process.
- Breach any of these Terms & Conditions.
In such cases, the Model will become liable for the full standard cost of any treatments already received or scheduled, at the Provider’s discretion.
10.2 Model’s Right to Withdraw
The Model may withdraw from the Programme before any treatments take place with no obligation.
If the Model chooses to withdraw after treatments have begun, or refuses consent for content use once treatment has started:
- The Model agrees to reimburse the Provider for the full value of the treatments received, at the Provider’s standard pricing.
- Any complimentary treatments or future sessions will be cancelled.
- The Provider reserves the right to retain any photos or videos already captured up to that point for marketing, training, or documentation purposes, unless otherwise agreed in writing.
11. Eligibility
- Participants must be 18 years or older.
- Participants must complete a consultation and be assessed as suitable for laser treatment.
- Participation is not available to individuals who do not meet the eligibility criteria (as per eligibility criteria given during application process and available on our website)
- We reserve the right to decline or withdraw participants if they are found unsuitable for treatment at any stage.
12. Appointment Times
Treatments are provided on a model basis, which may involve longer appointment times to allow for filming and photography.
